We do Immigration medicals-when making an appointment, please mention this is what you need.
You will need to arrive 15 minutes before the time of your appointment to make sure your details are completed on arrival.
Bring four passport size photos, your passport and the immigration medical forms.

form 1007 is the one you need.(printed in A4 size).
Look through the form before you come, so that you can give some thought to your answers.
Part of the medical will be done on your first visit here and the appropriate blood tests and chest X-Ray will be arranged.
We will see you again, usually in about 3 working days to complete the medical for you and enclose the results and discuss further tests if this is appropriate; the medical report will then be ready for you to take away at the second visit here (this will be arranged when we see you).
The total cost of the medical examination here is $200 here payable at the first visit.
The cost of the blood tests, urine (around $100) and chest x-ray (around $80) are separate and will be charged to you by the lab and radiologist.
After the medical you will also be registered at our practice and we will be able to help with your future medical needs in New-Zealand. The cost of further consultation is substantially less than the Immigration Medical.
